Passport Reminder

Reminder to Passport Applicants


 
•  Applicant should file his application where he/she is physically present.

•  Personal appearance by the applicant is required. For minor applicants below eight (8) years old, either parent must file the application.

•  Passport photos should not be the same as in the old/expired passport. Applicant must be in decent attire. No sleeveless blouses and plunging necklines for women. No lettering on shirts for both men and women.

•  Application may also be filed through the nearest Honorary Consulate or through mail/courier.

•  If application was sent thru mail/courier, the applicant will shoulder the courier/mailing fees. The Philippine Embassy is not responsible for any delay or loss during transit by mail.

•  Please allow two (2) days processing time and issuance of new passports for applications personally filed at the Philippine Embassy.

•  Passports unclaimed after six months from date of issuance will be cancelled and disposed of per Department Order No. 37-03.

A. RENEWAL OF PASSPORT

1.Personal appearance

2.Duly-accomplished Application Form

3.Three (3) 4.5 cm x 3.5 cm photos, colored or black and white with plain white background, and taken within the last six (6) months.

4.Old or Expired Passport

5.For married women who would like to adopt the surname of their husband in her new passport:

a. If married in Greece or in Cyprus, Report of Marriage duly registered at the Philippine Embassy, Athens, Greece

b. If married in the Philippines, Marriage Contract (MC) on SECPA (Security Paper) from the National Statistics Office NSO and duly authenticated by the Malacañang Legal Office and the Department of Foreign Affairs (DFA)

6.For women who wish to revert to their maiden name in their new passport on the following grounds:

a. Death of husband: Death Certificate of husband issued by the NSO and duly authenticated by the DFA

b. Annulment of Marriage from Filipino husband: Annotated Marriage Contract issued by the NSO and duly authenticated by the DFA

c. Divorce/Annulment of Marriage from Foreign spouse: Court Decision in Greek including the dispositive decision granting the divorce/annulment and its English translation.

B. FIRST-TIME APPLICANTS (FILIPINOS BORN IN GREECE OR IN CYPRUS)

1. Personal appearance. If applicant is below eight (8) years old, personal appearance of either parent will be sufficient.

2. Duly-accomplished Application Form

3.Three (3) 4.5 cm x 3.5 cm photos, colored or black and white with plain white background, and taken within the last six (6) months

4. Report of Birth duly accomplished at the Philippine Embassy, Athens, Greece.

C. REPLACEMENT OF LOST PASSPORT

NOTE: There is a clearing period of fifteen (15) working days from the date of application prior to the processing of application for a new passport.

1. Personal appearance

2. Duly-accomplished Application Form

3. Three (3) 4.5 cm x 3.5 cm photos, colored or black and white with plain white background, and taken within the last six (6) months.

4. Affidavit of Loss duly-notarized or signed before the Consular Officer stating the circumstances surrounding the loss of the passport.

5. Police Report with English translation

6. If lost passport was not issued in Athens, Greece:

a. Confirmation of issuance from DFA, or other Post which issued the passport

b. Birth Certificate (BC) on SECPA (Security Paper) from the National Statistics Office NSO and duly authenticated by the Malacañang Legal Office and the Department of Foreign Affairs (DFA)

c. For married women, Marriage Contract (MC) on SECPA (Security Paper) from the National Statistics Office NSO and duly authenticated by the Malacañang Legal Office and the Department of Foreign Affairs (DFA)

d. Supporting documents establishing the identity and Philippine citizenship of the applicant may be required if the identity or citizenship is doubtful

e. If available, photocopy of lost passport or other previously issued passport
