Lost Passport

1. Passport copy (if Available)
2. Birth Certificate from NSO authenticated by DFA-Manila (original should be submitted to Phil.Embassy in Athens).
3. for Married Applicant. provide marriage certificate from NSO authenticated by DFA-Manila ( original should be submitted to Phil.Embassy in Athens)
4. Police Report (should be in English) and make a photocopy.
5. Fill-out the Affidavit of Loss Passport form (provided by Phil. Consulate)
6. Fill-out the Passport Application Form (provided by Phil.Consulate.
7. Pay the said amount.
8. Applicant will wait for further advice from the Phil.Embassy for the issuance of the new passport.
